Tool: Scheduling Workflow for Walk-in Patients and Example Vaccine Screening Questionnaire

Tool includes an open scheduling workflow for walk-in patients and example vaccine screening questionnaire used in mobile immunization clinics. The scheduling workflow presents a comprehensive, step-by-step flow chart for training clinic staff on the e-check in process for walk-in patients. The example vaccine screening questionnaire, which patients fill out on a tablet when they are waiting for their appointment, includes general vaccine screening questions as well as screening questions specific to COVID-19 and the flu. Once the questionnaire is filled out, the nurse is automatically notified and the patient is able to receive their vaccine. Both the scheduling workflow and screening tool have allowed the Clackamas County Public Health Division to expedite the patient check-in process by alerting vaccinators upon the patient's arrival and allowing for real-time charting by nurses.
