Survey can be used to understand a person's attitudes and beliefs regarding flu and COVID-19 vaccines. Health departments and community-based organizations can use this survey to understand vaccine self-efficacy within their own counties. The 18 items on the survey include questions about perceived effectiveness of vaccines, trusted sources of information on vaccinations, and factors that impact an individual's decision to get a vaccine. The survey is available in PDF and Word Document formats.
