Toolkit: Transforming the Narrative: Health Equity & The Community

Toolkit offers resources for building cross-sector relationships and strengthening grassroots power to advance health equity. The toolkit includes a case study of collaboration between Local Health Departments and Community Power Building Organizations to reduce exposure to Adverse Childhood Experiences, or ACEs. It also includes conceptual frameworks and exercises to effect change in communities, such as organizational capacity assessments, root cause analyses, and power mapping.
