Tool: The Program Sustainability Assessment Tool (PSAT) to Assess and Plan for Sustainability (English, Spanish)

The Program Sustainability Assessment Tool (PSAT) allows health departments, community-based organizations, and health care providers to assess their own and their program's capacity and sustainability. Sustainability is assessed across 8 domains, including: funding, program adaptation, and partnerships. The resource also contains a report on how programs can use the PSAT to identify its strengths and challenges and engage in sustainability planning.
