Tool and Factsheet: Screening Checklist for Contraindications to Inactivated Injectable Influenza Vaccination (English, Spanish)

Tool offers a pre-vaccination screening checklist to be self-administered by patients or caregivers for a child or adult who is receiving an inactivated injectable flu vaccine. An accompanying factsheet for providers helps evaluate whether the person should be vaccinated based on the checklist responses.
