Survey: Measuring COVID-19 Vaccine Experiences (English, French, Spanish, Swahili)

Survey titled, "Partnering for Vaccine Equity", includes questions to learn about people's overall experience with COVID-19 vaccines, including attitudes and intent to be vaccinated, barriers to vaccination, and vaccination uptake. The survey, developed by The Linn County Public Health Department and available in four languages, also includes questions about respondents' experiences with health care unrelated to COVID-19. Health care providers and community health workers can administer this survey to understand people's vaccine attitudes, knowledge, and utilization and plan outreach and services accordingly.
